The mother is likely the stepmother of the boy and the narrator is the natural daughter of the mother and thus, the boy’s stepsister.

My theory: it’s likely the mother killed her stepson over the realisation that he will eventually inherit a significant portion of the father’s wealth and/or assets and her own daughter will get a smaller portion.
