Hughes Airwest Flight 706 was a flight between Los Angeles and Seattle on 6 June 1971 with 49 passengers and crew. The flight planned to make many stops along the way.
Flight 706 reached 15,000 feet several minutes after takeoff. Also in the airspace was a military F-4 Phantom II aircraft flying to Idaho. At 6:11pm local time, both aircraft collided into each other. The F-4's tail sliced through the cockpit of the DC-9, causing it to separate. The DC-9 then plummeted to the ground and crashed, killing everyone on board. The F-4 also lost control, and whilst the back seat passenger managed to eject and survive, the pilot of the aircraft died in the crash.
The collision was attributed to the fact that the commercial airliner was following IFR rules and the fighter jet was following VFR rules, which required aircraft to 'see and avoid' other traffic. This method of traffic avoidance was deemed unreliable at times. ATC technology was also limited at the time in effectively separating aircraft.
