While ATC was clearly shaken (but staying focused and doing EVERYTHING right) because he realized he was nearly in a redo of the Haneda collision, I'm amazed by the professionalism of BOTH pilots: the SW guys going "no worries" while fully aware of what their next half hour will look like, and the AMF guys going "got it" upon receiving their "turn right, turn right, call tower" instruction. It is just a pleasure to see a perfect example of how a (serious) incident is dealt with.

It's really a grey area, as Amflight wasn't issued a LAHSO clearance prior to landing. They cleared SWA to takeoff on an intersecting runway before Amflight was assured off the runway. They could've missed the first exit and didn't want to stop on an active runway, so they decided to keep it rolling. Not trying to defend anyone. But this is where the Swiss cheese holes lined up.

A good example of why it's so important to have properly staffed towers, too. Lots of collisions in the past involved controllers who had to move between posts and so weren't able to resolve situations that arise after they gave clearances.

Great job by the Southwest pilots being aware of their surroundings. Reading between the lines, it looks like they were well aware that another aircraft was near the intersection (it would have been right in their line of sight as they looked down their own runway), and were paying attention to see if it turned short of the intersection or crossed into their runway. It looks to me like they were already rejecting takeoff by the point where the controller started calling "hold, hold, HOLD", several seconds before the controller actually cancelled their takeoff clearance. They just had the sense to stay quiet about it for a moment to avoid stepping on the controller while he was trying to sort out the other plane.

I appreciate ATC that can pay attention as much as he did...he said he saw the plane turning for C6. So he at least visually observed expected separation

It's clearly ATC error if you look at the books. 7110.65 3-9-8 b. says to "separate departing aircraft from another aircraft using an intersecting runway" by a couple of criteria, the closest of which is for the other aircraft to have "completed the landing roll and is observed turning at an exit point prior to the intersection". If the animation from the video is accurate, ATC gave takeoff clearance before the AMF started turning. Moreover, you can't suddenly turn a taxi instruction into a LAHSO.

Well, the take off clearance was given before he freed the runway where it landed. Maybe it was too early. Could have been a brake problem for example and then intention of exiting at C6 was no longer possible.

Couple of questions about this one. Was 1943 unable to decelerate enough for the assigned exit? Is that why turning at 16knt is mentioned? Also, until they're clear of the runway, isn't the full length their's, so the other plane shouldn't have been cleared?

I'm not sure 1943 is the fault here if that's all the case. This feels more like a controller trying to save 5-10 seconds, rather than ensuring the crossing runway is clear?
