It's not whether Trump is unable to discharge his duties in the office of President, the question should be "Is Trump discharging his duties in the office of President?" If trump has been delegating his work to others and they are causing the havoc, trump cannot hold his hands up and say that he knew nothing about it and he's not to blame. If he delegates his personal presidential duties to others, he is responsible.
