The name Team Plasma makes a lot of sense. It's not just randomly chosen.

Plasma is the fourth state of matter, in which the particles are completely separated. Examples of Plasma are Fire and Electricity.

Team Plasma wanted to separate Pokemon from humans in Pokemon Black and White by catching either Reshiram or Zekrom, who's secondary types are either Fire or Electric.

However, Team Plasma was separated in Black and White 2 into the peaceful Plasma and Neo Plamsa.

The name Team Rocket comes from the Rocket Reds, a group of kids that bullied Satoshi Tajiri, the creator of Pokemon, as a kid. This is backed by the fact that the leader of Team Rocket, Giovanni, is named Sakaki in Japanese, after the leader of the Rocket Reds, who killed Satoshi's pet Tadpole.

It's heavily implied that after his second defeat, Ghetsis lost the will to go on as well as mentality to lead (there are hints that he's already mentally unstable; his final lose finally caused him to snap). As a result, Ghetsis can no longer lead Team Plasma, much less take care of himself, hence he can't return.
