I'm pretty sure one of the Journeys episodes has an episode based around how a Ditto is on a film set and is too stressed to transform properly, the movie being made I think is about two Gardevoirs

Ditto is supposed to be represented as the goofy transformation pokemon but they decided to make it a plush and now they sell like hot cakes

One game I recall uses this. The original Pokémon Snap would have Ditto disguised as other Pokémon, but they had the goofy faces.

Tbf that's just how a lot of Kanto Pokémon were handled. The experiences that 1 specific Pokémon of its species went through in the anime or whatever early media it was featured in went on to become its canon lore.

This creates a lot of plot holes and raises so many questions though because this implies a Pokémon like Cubone must experience becoming an orphan on a species wide level.

It's not only in the game where ditto can transform perfectly, it also works like that in the manga

If i recall correctly ultra sun and moon has the 5 ditto challenge or something like that where you need to track down 5 dittos that are hiding as humans with ditto faces

I think most Dittos that struggle with transforming always have the face, but those who are better trained in the skill don’t usually do it

What if most ditto? Can’t transform perfectly at least in like the TV show. But with enough trial, they can perfectly replicate the face.

The smile isn’t in games because it would be a monster to code. They would have to edit every Pokémon that can possibly be brought into the game and create a model for them with a different face, just for a gag.

Nobody would take the time to do that.
