On the name and date thing, it's common to have it MMDDYYYY however it may be YYYYMMDD especially with Government paperwork, also for name it's again common to do First, Middle, Last, however it may be Last, First, Middle Initial, again mostly with Government paperwork but it depends, that said it's going to be indicated on the paperwork on how it should be written if not it's best to ask, so don't just assume it's only going to be one way all the time, and make sure to look on how it's indicating to be written, this will save you a lot of headaches.
