Let me ask a few questions:
1. If the incision is too wide to introduce the finger how can we close the wound to prevent air leak?
2. How can we direct the chest tube to prevent it from going into the fissure?

What about pisterior to the mid axilary line? Is there a risk?

But the literature says the lowest margin is at a line crossing the nipple. Here it is low down!?
