Transitioning from Adolescent to Adult HIV Care

This session is on transitioning from adolescent to adult care among HIV infected individuals. This presentation takes a look at the difference between youth who are perinatally infected and behaviorally infected with HIV relating to the scope of diagnosed HIV cases, clinical issues, different HIV care models and life skills needed to transition from adolescent to adult HIV care, as well as best practices in transitioning by the State University of New York (SUNY) Downstate Medical Center.
