When you work for an agency or group practice, you often simply use the intake paperwork your employer asks you to use. But how do you know what questions to include in your intake form when you’re running your own practice? In this video I break down some items to consider including when developing your initial intake form.

Thank you for your information...very much appreciated. I have a question that puts a spin on the topic. My question is: How do you record your documentation- online platform like TherapyNotes, Google Docs, or some other method?

This is very helpful, thank you! I was wondering what the structure looks like in an actual intake session (first paid session). Are there any recommended forms/questions for how to explore this context more and work towards collaborative goal-setting? I would think the first session focuses mostly on collecting information and establishing the therapeutic alliance. Thanks again :)

THANK YOU FOR THIS! Is the intake form mandatory in private practice? Or do you just need the consent form? Thank you.

I am starting my own occupational therapy private practice and find your videos very helpful! Do you have any recommendations on what software to use to create fillable intake forms (i.e. Adobe Acrobat, PDFfiller etc)? Thank you!

I haven't seen any recent videos about the other forms of documentation like notes. Are they titled under something else?
