Just curious, was the ATC at the beginning a recording, or someone giving you actual clearance? As it said cleared to take off runway 04, yet you took off from runway 12. Also at one point one aircraft had it's gear down, yet flying at speed which would in normal terms, be in excess of the recommended speed for gear to be down.
