He pulled her out and they were saved, but she ran back to the car and got killed. She had gone back to get the high school ring he gave her and they found it clutched in her hand. Guy’s did that back then, gave their high school ring to their girlfriends.

She ran back to the car for his high school ring, which was a fatal sacrifice. Lots of teenage "death" songs in that era. "Tell Laura I Love Her, " "Last Kiss, " and "Patches" (the 1962 song and not the 1970 song with the same title) were also among the biggest and most morbid hits from that time. Teenage angst was a big thing in those early rock n roll years.

In the song he did pull her out from the wreckage but she went running back and he ran after her and she reached in and got inside the car to get the ring and he was not able to pull her out before the train ran over the car and killed her that's why at the end of the song he says I'm sorry he was not able to save her in the end

So many oddly dark songs from this era, Tell Laura is another. Romeo & Juliette complex I think because happy nonsense was being pushed on kids so hard that these songs had a morbid attraction
