Yes, it works till about gold. After that, you need to use high ground and cover to pressure out cooldowns before you dive, or you will just get blown up. Also, the better you are at being patient and waiting for your dps to be in a position to help, the easier it is to confirm a kill.

Knowing how is the first step! Diving is especially strong against heros that play further away from the team like snipers and midrange like Cas and soldier. Make their tank panic and let your dps finish them off then go push

It's a bit more complex than that.

The best way I can explain is dive tanks use to prospect of mobility to push their turf. So, the first few engages are going to be "fake" engages where te priority is to take damage(farm nano boost), deal damage(farm winton ult), and most importantly, take aggression away from your flanking DPS so they can set up at an advantageous position.

Once the cards are all set(Ana is close to nano, Winton almost has ult, Tracer is staged in their back line, your previous engages warded off enemy DPS from a high ground etc etc, ) then you go for a deaper dive.
