At 2:12, this error can also happen if there is no DVD drive installed in the GameCube, or in the case of the GCLoader (and clones), it could mean a faulty SD card extender (people use them a lot).

At 2:35, this error can also happen if you disconnect the controller board, which contains the battery to keep the time. This is especially important in games like Animal Crossing. And if you select No on that error screen, you will still be sent to the main menu.

For number 2 (An error has Occured) There are actually 3 different ways to get the error. The first two (obviously) are correct. The third way is if the fan in your Nintendo Gamecube is either not present(for some odd reason) or it’s broken. If you do somehow manage to miss that error, it will give you the disc error
