Well the infection was contracted by fleas, the infection rate among humans wasn't so high, the puss liquid being the most contagious. But people knew to keep away from that.

Fun fact: The word "quarantine" can be traced back to italian venice around this time period. "quarantena" meaning forty days. In which they quarantine the passengers, crews, and all its cargoes of any ship that docked in venice for forty days before allowed to enter the city. Despite its strict measure. Venice still lost 30, 000 of its inhabitants. About 30-40% of its total population.

This is a very mathematical/statistical view on the time period.
One has to remember that this casualty rate is a modern estimate, as the number of dead literally became too high for people to count back then.

Likewise, with the "coinflip" analogy, this isn't exactly true. If you were to travel between cities and towns, you'd see places where not one person survived or places that were somehow left untouched by the plague. Most places, of course, had a mixture of survivors, infected, and the dead.

Lastly, the Black Plague didn't really "end" with the Middle Ages. It kept dying down and reappearing well into the late Renaissance.
