New hope in lung cancer treatment with 4D-CT scan
What is 4D-CT?
Four-dimensional computed tomography (4DCT) is a type of CT scanning which records multiple images over time. It allows playback of the scan as a video, so that physiological processes can be observed and internal movement can be tracked. The name is derived from the addition of time (as the fourth dimension) to traditional 3D computed tomography.
How 4D-CT helps in lung cancer treatment?
The problem in targeting lung cancer with radiotherapy is movement of the tumour with respiration. Lung tumours can move unpredictably with respiration even up to 5cm. Various motion management techniques have come up with advent of time to sort out this issue. 4D-CT scan is one such technique, which can reconstruct the three-dimensional lung tumour volume taking into consideration of tumour motion with time. This allows the Oncologist’s to preciously check tumour motion and target the tumour with radiotherapy accurately. Therefore, 4D-CT based Image Guided Radiotherapy (IGRT) planning not only expected to target the lung cancer precisely but also reduce the dose to other normal organs (lung, heart, oesophagus, spinal cord) and thus reduce the side effects from radiotherapy treatment. It can be used for treatment of early stage lung cancer as well as for advanced stage lung cancer.
What is gating?
4DCT will usually involve a gating technique, such as breathing tracking, so that image acquisition is automatically triggered at set points. This gating can also be applied at treatment, where the radiotherapy beam is only switched on at certain points in the breathing cycle (as in the deep inspiration breath-hold technique).
Is there any other cancer where 4D-CT is used?
Apart from lung cancer 4D-CT has shown to be very effective in treatment of liver cancer, pancreatic cancer, oesophageal cancer etc.
