So my frustration is that neither supraclavicular nor axillary block provide a reliable block for ulnar nerve transposition. The entire arm is numb except for the incision site. I’m assuming I’m missing the median cutaneous nerve. Is this nerve usually blocked with axillary block? If not, what additional block can I do for post op analgesia? Or is it just my technical shortcomings? It def could be my technique, although my blocks usually work for other upper extremity procedures.

Why is it OK to inject local anesthetic in the sheath at this level, but should you stay out of it at the supraclavicular level?
