I like how the hand placement is symbolic for all of the family members -
1. The mother is holding his neck, which signifies that her behavior and actions lead to Shigaraki feeling suffocated, as she never does anything to prevent his abuse;
2. The grandparents are holding his upper arm, which would suggest they are draining him of his strength;
3. The little sister is holding his lower arm and hand, which is in fact a sign of compassion and care, but also fear and a tendency of holding him back;
4. The father, who is grabbing Shigaraki's face, which clearly is a symbol for how the father abused him and traumatized Shigaraki, completely changing his world view with the palm reducing his vision;
5. Lastly, we got Nana, holding the top of his head, which shows tha she was always in the back of Shigaraki's mind, as he never got to meet her in person. She symbolizes good conscience, aspirations and dreams of being a hero.
