I hope this investigation includes investigating the failures of law enforcement?

You want to prevent other shootings then maybe act on the information you get before something happens. Multiple sources tried to alert authorities about this guy and they didn't do a damn thing. Now they are making a commission to cover their mistake and pass the buck.
