For the people wondering, he in fact forgot, and there’s a Lore reason behind that explains it.

When Guy was raised by his father Duy, he always told him to never forget those who had courage and great moral values of justice; which in Guy’s adulthood manifested as his youth spirit and warnings to only use Kinjutsu when you have to protect someone “I don’t think that being a Ninja is about defeating an enemy, but about protecting what you care of” the only reason of why Guy did remember Kizame’s name is because he decided to sacrifice himself rather than letting his allies down and what he believed was right; and in a certain way, that’s what Guy believes in and the reason of why he says: “He was a worthy opponent! A man who lived and died like a real shinobi! Kisame Hoshigaki! I'll remember that name for the rest of my life!”.

Guy found redeeming qualities in Kisame such as Loyalty, Determination and Will, which caused him to remember his name; but he can’t remember the name Madara Uchiha because he can’t find any reddeming quality in someone who caused a World conflict, destroyed millions of lifes for decades, liked to humiliate their enemies, seeked power to control other people, and that wanted to bring a fake paradise in which everyone would live a delusion.
