It depends of the diameter of the shaft there is suitable cutter that fit the grooves, so you need more than one universal cutter, also there are more types of spline shafts

Simultaneously means at the same time, so if they're distributing the torque "left right left right" as you say, that's not simultaneously. The fact that all the inserts are on the circumference of a rotating spindle also indicates they aren't all cutting simultaneously.
