Hello i just left City Hall and they stated i can not write N/A for any of the boxes that i dont know answer for.

For me it was the city my Father was born as well as his DOB!

When printing out my ds-11 do I have to get specific type of blank paper ?

As long as you get a birth certificate that was issued after July 2010 you should be fine. Might want to get a new copy just in case.
