In nightmare mode you have to be careful with the motion tracker, probably that’s how it spotted you. In this case try to rely more. On your hearing. Or maybe it was close enough to hear the door or even lightly see you.

IF you held motion tracker it hears it, same mechanic as in lockers when you hold breath
