Uploading file with custom name using Flask and Python

Thanks for your great tutorial, I'm actually working on a project that really needs this, but I have two questions.
1)What if I want to upload more than one files, and want to force name each of them, how can I do that?
2) I tried the code on your Github, but I didn't find the "choose file" button, cause I want user to upload five files and I want they to click choose file button five times instead once. so do you have any ideas, thanks!
