Not mental, but neurological. The spice doesnt actually burn you, it just makes your body think its on fire, and then your body and brain reacts the way it would if you were actually on fire. So there is a real physiological response to what is essentially a simulated crisis. Not everyone gets the same kind of physiological response, or you can train your body to reduce the response, which I guess makes people think its all mental.
