To be clear, I’m not saying you should literally be 3 kobolds in a trench coat, just that you should use that “multiple creatures but one statblock” style for your multiple character’s character.

Just play as a regular multiclassed character and flavour it to a duo character. Maybe the DM gives you an extra action every so often if they feel like it. If I was Dm, I'd let 1 be a fighter and simply restore action surges between Initiative

I once (3.5) let a player run a character who was under a curse. A couple who were cursed to never meet, a male elf (wizard) and his human wife (fighter) because the curse forced them to switch out on a day/night cycle. Their motive for adventuring was to find a way to cure the curse.

There was something i did really similar to this. I wanted to create a character similar to the Lizard from Spider-Man. So, I made 2 sheets: an artificer alchemist gnome, and a beast barbarian lizardfolk. The main character is the gnome, who is used in almost all interactions. However, in combat, if needed, he changes into the raging beast for one minute, until the rage ends. His health stays at the same percentage either way. For example, if I'm at half hp as the gnome, then I'm half hp as the Lizard. I've tried it in 2 groups and it seems to work well, but can get complicated.

I played a campaign where one person played 3 gnomes in a trenchcoat. The way it worked was he only got one turn per round same as any other character. Each got their own HP pool but shared exp so only 1 could level up at a time. It gave a lot of options but he had to be very careful since all three were pretty weak.

This is very similar to a character I'm playing now :D Currently, I play an Aasimar Rouge/Warlock who also carries the soul of his patron and an Elf paladin. I don't get to play the patron but I do play the paladin along side the Warlock. They play as two separate characters with minor changes. They share an inventory, don't share a health pool, and don't share stats. The two characters hate each other so working together in combat is off the tables. Even then, in combat it takes a couple turns for either one to take control and I have to roll contests against each other. I actually really enjoy this mechanic because it brings roleplay into combat :D
Despite all that, they both are my favorite characters I've ever played, and my DM is handling both the mechanics and their story SO well

There's also the option of the various "pet" classes and subclasses. Those pets are characters in their own right at least narratively, and if your pet can't already speak (like a Chainlock's Familiar usually can) I don't feel like it's be the biggest deal to ask your DM "hey, can my pet talk like a person so that I can roleplay as two characters but not overburden combat or the general balance of the game?"

The way i would do it with the piggy back method is the big characters only skill is high durability and throwing. And he only yeets the smaller player at enemies, the smaller character being the only one to attack. Its like a mount if a mount threw you at things that can kill you

Or you could be physically 2 characters fused together. Like through some weird science and magic experiment, someone tried to pull a Frankenstein and make 1 new body with 2 old ones. And the 2 body’s minds now share the body.

If you want to stack stack with players, put a little dwarf on the big barbarian

I've played a character that was a gish, a warlock to be specific. I played them as a couple who were fused together via a curse. One was good and the other evil and one was a caster and the other a martial. I played one at a time but they talked to eachother occasionally. The idea was simply separate playstyles for the same ability suite. It worked great!

I had a player ask about being a gnome riding around in a Warforged like Iron Man. I said yes, but here is how it'll work: Gnome is one class, but then climbs inside, and is now another class. They get 2 characters, 2 classes, but the Warforged can't move without the Gnome inside, so the action economy for him is still "just 1 character"

Battlesmith is probably what suits this style of play best. You could be a kobold who piggybacks on his steel defender which I believe can be medium sized if I’m not wrong.

As someone who plays a duo multiclass. I specifically have an ability were we detach from eachother an get 2 turns on combat and 2 health an resource pools, but when together I get an affinity boost to my magic and we share the same health an resources.

One technique I have used is I had a RogueWarlock duo/multiclass that were chained together with magical shackles where in, if one of them dies, the other dies immediately, and if the shackle is broken, they both die. They pretended to be a super close crime duo, but ultimately they were both planning to back stab each other and the Patron of the warlock was pointedly setting them up to do so. Ultimately, the rogue did kill the Warlock, inheriting his levels, then betraying the party to a messy end. It was REAL good drama. 10/10 would recommend life-linked chains for multi-character characters.

I would say it is just flavour while mounted but upon the dismount you choose one of the pair and run with that one same hp and ect. But you don't have access to the other's abilities until reunited as one.

I remember a friend taking about he act has both the 4th and 5th party members. His character switch between 2 character under certain conditions. 1 was a barbarian and the other was a sorcerer. They were brothers sharing a body. At least soul wise. Each class was mutually exclusive the character A or B. So barbarian wasn't magical talented and bearly can use magic, but the other can. They were aware of each other existence, but not what they do when one was in control. The only way to switch between character was for them to be Incapacitated. This can be done by having their hit point hit 0 and automatically switch to the other with 1 hp or make them fall asleep which automatically wake up and switch out 1 for the other. It should be noted that if both get sleep they stay slept till awaken. Or both get down then death saving throw are roll. Their group necromancer eventually made sleeping pill that act instantly, but that a limited resource with side effects if taken too many time a day.

The down brothers will stay down until the end of the encounter or the other brother is swap out for the down brothers then death save are not roll but a fail death save will be made if they take damage. At that point just get the down brothers up.

I've done a similar idea with pet classes multiclassed with the echo knight fighter to get 3 characters. The first version was a battlesmith whose pet was reflavored to be a sort of frankenstein's monster, and his wife was a ghost (the echo). The idea was that the man and his wife were supernatural detectives. She died in child birth, and the ghoul is their adopted son.
the character itself operates as it normally would, with the echo being more about positioning, and using the sentinel feat for "tanking". You attack with either the echo or the character, then bonus action attack with the "steel defender"'
The second version was 3 brothers. A halfling, an imp, and a ghost. The halfling was the only one who survived their abusive warlock parents. The imp was sacrificed to give them power, and later came back as an imp, and the ghost is just a vengeful spirit. For this one, I took 3 levels fighter, 3 levels warlock (for the imp) and the rest in rogue.
