Karkat was always aware of her feelings. He thought she was cute. He just never reciprocated those feelings and always thought she could do better. especially since she had Equius as an amazing moirail, despite him being very controlling of her actions. If I had to guess, Karkat most likely didn't want to follow in the footsteps of his ancestor and accept the conditional love that is being given to him. He always had to work hard to get where he is, being given a lusus despite his mutation and showing how much he has accomplished alone. Having Nepeta would probably make it hard for him to protect others since she would have to become a priority.

I know that a lot of people feel bad for Nepeta because her feelings weren't reciprocated, but I feel bad for Karkat. He didn't want to tell her he didn't feel the same way, so didn't let her down, but couldn't force himself to pretend to like her either. He shouldn't even have to choose between crushing her or pretending, as pretending shouldn't have to be an option. Nepeta should be open to hearing Karkat's side of the story, even if it isn't the side she wants to hear. *No one* is obligated to return someone's feelings, or pretend to. People feel the way they feel and both the crusher and crushee should have their feelings respected.
