Honestly, it was all the fault of the airline that did not properly train the pilots to fly the MD-81 and the MD-82 that crashed in Venezuela.

I watched the episode on this crash, anti-icing wasn’t done but I don’t remember why, it caused the plane to stall, and pilots did everything wrong and never recovered, killing everyone.
