What should be done if no labour signs are seen at 38 weeks of gestation? - Dr. Teena S Thomas

Usually at the end of 40 weeks, we expect the delivery. But only 4% of the people deliver exactly on their date. Rest can deliver anywhere between 37 to 41weeks.So if there is no expected signs of labour till 38 week, we need not act immediately for anything about this. We need to counsel the patients to be carefully watching baby movements which is very important because the entire progress and the further wait on the pregnancy depending on the mother’s feeling on the baby movements. So if there is any decline in the baby movements, then we gave to do a non stress test. At 38 or 39 weeks also we can ask for non-stress test which is an indication of a fetal well-being. So waiting till 40 weeks is usually required but after 37 weeks, the baby is ready to be delivered at any point of time, but we usually do not induce about 38 weeks. So what we usually do is be sure about the baby’s well-being. If required, get a non stress test done or biophysical scan done as per the requirement. Counsel and make the patient aware as what all signs of labour can be including pain, or nay leaking or decreased fetal movements or anything. So make the patient aware of the signs. Last thing what we can do is do a membrane sweep and try and induce a little bit in labour. But otherwise that is not required and can wait upto 40 weeks. Beyond 40 weeks, we can wait for a couple of days but be sure about the baby movements and the non-stress test. But it depends on the choice of the gynecologist and the patient to wait beyond 40 weeks. But this is what we do, beyond 38 weeks, counsel the patient for baby movement. Do counsel the patients for membrane sweep if required and wait till 40 weeks.
