It is possible to save both Anna and the children. Taken from steam forums:

To avoid this and save both the children and Anna, start Ladies of the Wood as you normally would up until the part where you help Johnny regain his voice. Then when he tells you to follow him back to the orphanage, leave instead and go to the Whispering Hillock and speak to the Tree Spirit. Agree to do the ritual. You should already have the feather. Go and collect her bones and then find a black horse. Return with everything and begin the ritual. She will then possess the horse and leave.

Go back to the swamp and resume following Johnny. Speak with the Crones, agreed to do as they ask. Get the dagger from Anna and go to Downwarren. Speak to the Ealdorman and get his payment (the ear) and return to the bog. Speak again with the Crones and tell them you freed the spirit. They will now know that the spirit freed the children and not Anna, and she won't be cursed. They'll be angry at you for deceiving them but they'll keep their end of the bargain and tell you what happened to Ciri. End of quest.

Unfortunaly this doesnt save the kids, doing these steps wont maken them appear in the orphanage in novigrad.
They only appear in the orphanage if you release the tree spirit in the quest (you get a special line from the tree spirit that shell save the kids). If you do it before meeting the crones or during Johnnys quest you wont get that special line and the kids wont appear in the orphanage.

Currently there is no way to actually save both the kids and Anna.

Good ending? "She who knows" is free, the kids are still eaten and Anna still lost her mind. I think this is the definitive worst ending.

I decided to save the orphans, Baron was abusive to everyone, he was hurting and killing his men, beating his wife in a drunken rage. ann was a angry with baron because he killed the man she was cheating with and would sometimes try to kill him or herself. The villagers would willingly sacrifice their kids to the crones, so to me the only true good decision is to save the kids. Who’s only crime was existing.

The consequences of doing it this way is :
Lost of life the villagers of downwarren
Lost of life the kids at the swamp (you didn't asked the horse to save them)
but
Anna live
Mythical horse live
you pissed of the crones.
thus with the intention of saving mythical horse and anna you killed a village and the kids.

I didn't go to the tree early but still got the same "good ending for Anna". I killed the tree spirit because I think it kills the villagers sometime after it is freed. But that means the kids died to save Anna and the villagers as others have pointed out.

Ohhh yeah this was the one part of the game where geralts voice was a bit different.

You don't have to go to tree first. I have done the ending where she lives and Baron takes her to find a cure. It is in the dialogue you choose. Hell, funny thing is, in the over 1k hours I have played, I have never gotten the curse ending. That was a new one for me. But that is what I love about this game and why it is such a masterpiece.

The thing about this quest is, Geralt wouldn't trust the tree tbh.
Just like he said, if the kids are in trouble then he'll save them himself. He didn't think the situation would be as messed up as this.
At this point he only sees these people worshiping the ladies and how the kids are living safely in that "orphanage".

Thanks for this video, i tried it just now and it works. released the tree spirit first, then talk to Johnny restoring his voice etc. talk to the ealdorman of Downwarren (but i chose not to tell him that i freed the spirit cuz the less they knew the better lol) returned to the crones and there is this very interesting conversation where they actually realized that the missing children is our doing.. not Gran's (Anna) fault. After that continuing the story until we help release Anna (Gran) and she didn't turn into a Hag. The only downside is i saved the children, Anna (gran) from the Hag Curse, prevented Baron's suicide.. but the villager in Downwarren still have to die. well.. this is the best outcome for me. That's the number i can live with hahah

The problem with this path is that you freed an evil spirit that will wreak havoc on the people of Downwarren and the crones will kill and eat the children. Imho this is the worst possible outcome.
I've taken all possible routes and prefer to save the innocent children or to kill the spirit and save Anna and the villagers - though they're all not innocent, just apathetic to the evil surrounding them, and Anna is just as much a perpetrator as a victim.

Geralt's a witcher. Witchers' mission is to kill harmful monsters. So I killed the Tree Spirit and two of the three Crones. To kill the third Crone, you must get the bad ending where Ciri dies. Or you can get the good ending and hope Geralt or another witcher kills the remaining Crone. In my first play through, I got the bad ending so I vanquished the last remaining evil Crone.
