HMO plans, whether they are Medicare Advantage or non-Medicare plans, will generally restrict the enrollee to in-network providers and require referrals to see a specialist. But PPO plans (both Medicare Advantage and non-Medicare plans) do not restrict the enrollee to in-network providers, and do not require referrals to see a specialist.

With original Medicare, you can go to any provider that accepts Medicare Assignment, and also agrees to accept you as a patient.
