HIMSS Interoperability Showcase - Managing Chronic Pain and Mental Health

Our patient, Halley, has extreme chronic pain and mental health issues. Halley, a 46 year old unemployed Caucasian female, has been receiving outpatient mental health services for her bipolar disorder. As part of her care plan to address her bipolar disorder, she was prescribed Lithium as well as individual therapy sessions to help with coping skills. During her psychiatric evaluation it was also discussed that Halley had been involved in a car accident that left her with severe chronic back pain. Halley seeks drugs at various clinics in and near the university she attends for pain management. Halley receives opioids and eventually overdoses and is rushed to the Emergency Department (ED). Her overdose is reported to Public Health. At discharge, her case is assigned to a case manager. During outpatient treatment, Halley’s case is identified for a clinical trial giving her access to a mobile app that connects her to a broader care team to help her make informed decisions about her health.
