Not sure if this is just an American deviation, but it seems like age can be placed in multiple places in the order:
"Have you seen her new little black dress?"
"They carefully stepped into the cold, ancient, sloping cavern."

Of course, one of the ways "old" can be used isn't *really* an age descriptor, but some American vernaculars include tacking it onto certain other adjectives to add emphasis (although the "d" is always dropped when I've heard it, so it's definitely informal):
"There's a great little ol' restaurant that just opened over on the south side."

It's a very helpful rule indeed, but the order of the vowels take precedence. It has to be /i/ /a/ /o/ necessarily. It's the only situation that forces us to break that order. For example, "big bad wolf" sounds more natural than "bad big wolf".
