According to Funimation, Christopher Sabat does have a hard time dividing his roles between Piccolo and Vegeta. Piccolo says Goku, Vegeta says Kakarot, so it’s easy to mix them up. For the manga, it was a translation error. Hope this helps anyone who wanted context :)

1 and 3 actually make sense with context. The rest were inexcusable errors and I have no idea how they were let slide. I actually have the manga volume where he says "Goku". It was a baffling read.
