'I am tired and I shall go to the hospital.'

If you are tired (and not ill), then you better go home and rest instead of going to the hospital.

The second sentence is a great example of a threat, though we recommend you don't tell people that or you may get arrested.

Shall and will are often very interchangeable, it's just how sure you are, or how much emphasis you want to put that will decide the use.
