Tolkien never says who he is. In D&D, the same trip exists in Dragonlance as Fizban, who is the personification of the god Paladine.

I believe Tom is the avatar/manifestation of Eru Ilúvatar. He may not even know he is Eru Ilúvatar - like Tom is the dreaming state of Eru.
