To everyone wondering how they survive the head stab, he's likely stabbing the cerebellum AKA the part of the brain that affects your motor control. He's essentially semi-paralyzing them, they can move but not well enough to escape or even struggle. Notice how she barely puts up any resistance when he's slitting her throat.
