Very nice video however I want to indicate a few points.

POSITION& PREMEDICATION

In underdeveloped countries it is often done without giving any midazolam or propofol however I believe it should be done with pre medication that will tame the patient into a cooperative mode and make it easier for him to bear it.

I/V LINE
For a patient in left lateral position the IV line got to be in the right forearm and for a right lateral position the IV line got to be in the left forearm. So it may be done in either left or right lateral positions depending upon the logistics of the facility.

Regarding position of the patient left lateral position is to be preferred as the scope has to be moved from the right hand in. As soon as the probe is into a oesophagus we change the hands and hold the hub of the scope in our right hand right hand, that makes things easier for rest of the procedure. However whether you use left or right lateral position the active or introducing hand has to be right hand for a right handed operator.
If the procedure is going to be of longer duration then it may be prudent to keep the head end of the patient at a slightly lower level in order to enable flow of salivary secretions in absence of suction. If suction is available then it may be done at 30 or 45 degrees head up.

I would need to be sedated no way. I had TEE done before my ablation a few weeks back. During my ablation I went into cardiac arrest and they did CPR intubation etc. I woke up intubated. I couldn't move cuz they gave me a paralytic. I couldn't talk with the tube. I was absolutely terrified. (After that experience no way could I ever do this awake). I had a pacemaker installed next day.
