The NTSB report says the pilot was planning to hover about 75 feet off the water and then land on the riverbank, but as the pilot throttled up slightly, the report says the helicopter quote "started to settle" and he opened the throttle to max power to try to stop it when it hit the water.

Three people including Clayton's brother, Jim, escaped from the helicopter and were rescued by bystanders on a boat. The 3,000-pound helicopter was recovered 435 feet from the landing pad in roughly 40 feet of water. The report says there was no fire or explosion.
