when he says "they already know", he means that camilla has an onlyfans which she uses youtube to promote, sometimes including him in the videos. since everything that's put on the internet stays on the internet, when employers look up his name, porn and porn promotions are probably what first comes up. this means that he's probably going to struggle to find someone who will give him a job or accept him in a college as he said. girls aren't going to want to date a guy whose related to a big name onlyfans creator which is also an issue. basically he's mad because his sister is getting all this fame and making thousands while he's stuck with nothing and a life that's ruined before he's even properly started it.
