What is the mechanical discrepancy?

His main problem is flexibility in his hamstrings and achilles. Lack of flexibility in those areas causes him to lean forward as he squats. That is why he loses the weight forward. Have him squat down as far as he can before he starts to lean forward then have him concentrate on pushing his hips and knees forward. Sitting you butt between your feet is a different feeling than a back squat. Overhead squats in running shoes won't help his mechanics either.

lack of shoulder stability/flexibility, can't get into a full squat because the shoulders won't extend "back"
