my uncle used to work for CN for decades as an accident investigator. A couple of things he told me that stood out. 1. The most common reason for an accident is poor track maintenance. 2. in his entire career, only once did the company apply the recommendations in his reports. It had to do with signage.

Fun fact: every single automobile carried on that train can no longer go to market. Even in the undamaged traincars where the vehicles may still be in perfect condition, as they've been involved in a derailment, they're effectively "totalled" and will be scrapped.
