This depends if it's Douma in character or bloodlusted.

If it's bloodlusted, Douma wins mid diff since he can just spam his ice attacks from a distance along with the clones that are almost as strong as him, all while Gyomei can't breathe or else his lungs get decinated

If it's in character Gyomei wins extreme diff since he can get a bunch kf early hits on Douma and trip him up, allowing him to start the fight with a huge advantage

Also Douma was holding back against Kanai, Inosuke and Shinobu, and holding back a lot, so that's why bloodlusted Douma wins mid diff

Why am I thinking like that? Okay, let me explain.
In another verse we always see on abilities characters and call it "power of characters". For example, Madara in perfect susanoo destroyed six mountains and people said: "it's madara strength".
