They usually use Hot pack in case of any musculoskeletal injury which is not always right. In case of any acute injury which has caused swelling or inflammation, cold pack application should be your first choice.

Hot pack should always be used in case of muscle spasm and stiffness which otherwise has not caused any swelling and inflammation.
