Im a biochemist and 13 year EMT. These sensors were NOT common cheap thrrowaway equipment like they are now. It's genuinely astonishing nintendo made this for a game. Those chips are almost certainly proper ones from a real pulse oximeter.

Also these things were BULKY back then. This is an amazingly miniaturized one for 98

I think "normal" is that the game gets more difficult as your heartrate increases, while "reverse" gets more difficult if your heartrate is low. Putting the default as "reverse" would make a lot of sense as the other would just be a positive feedback loop where you get stressed from the increase difficulty until you either lose or suffer cardiac arrest.

Also considering this is JDM I kinda expect the english translation to not make much sense.
