I found sally in door 23 so I gave her an item and the achievement appears and then when I hear rush sound I was like “oh god…” and then died

in the new content update, windowis replaced with a new entity model. sally is a little girl who is signalled by flickering lights and a giggle. sally then appears behind the window and enters, chasing you and dealing a huge amount of damage UNLESS you drop an item for her to play with or use the crucifix.
