It doesn't have to be a mod, you can use the custom model data feature from resource packs to add the discs & songs. Add a datapack with advancements that detect when a player uses the disc on a jukebox to trigger a function that plays the song. It's seamless. I've done it. The datapack can also change loot tables so your discs appear as chest loot or a special drop from a mob.
