Have you spoke with Mariana Evans about the piezo bone saw method?.. I would love to get her take on it

Kasey Li has actually been doing his EASE mid-palatal cut orally at least since summer of 2021. He probably is doing something very similar with a piezo saw. It's quite good that providers are doing this type of MPS cut because it gets you one step closer to being able to do EASE (other two main hurdles are the KLS expander is quite different, and his pterygomaxillary cuts are not easy either).

I have MMA surgery with genioplasty 10 months ago. My bite was normal prior to surgery. I do it for my sleep apnea with off course huge improvement in my appearance i would say. But Now my bite is edge to edge. In Normal bite, upper teeth should sit in front of lower teeth. My surgeon said because my lower jaw was advanced so much by 22mm (because i requested for “Ante Face” look), my upper jaw has high risk for relapsed as it might unable to compensate the huge advancement of my lower jaw. I might get underbite later on and i am not a person who really compliant with wearing the rubber band. My surgeon said i need to wear the rubber band for at least 4months to stabilize and maintain my bite after the MMA surgery, but even now post MMA 10months already, If i dont wear my rubber band even just for few days, my bite will relapsed back to edge to edge bite.

Now post MMA 10months, My surgeon has advised for another surgery called Anterior Maxillary Distraction (AMD). According to my surgeon, AMD is usually reserved for cleft lip/palate patient because they have congenital maxilla hypoplasia. My surgeon will cut my maxilla from canine to canine and I will need to wear an expander exactly like what you use for MSE. But instead of expanding my maxilla laterally like in MSE, AMD will move your maxilla forward, and in my case my maxilla will move forward even more on top of the already advanced maxilla from previous MMA surgery. After expansion complete, my upper jaw will be forwardly placed infront on my lower teeth and a gap will be formed next to my both canine. I will have to wear the expander for 6months so that the new gap will be filled in with new bones in which after that i will need dental implant to be placed on the gap on both side of my canine, not braces to close the gap like in MSE.

Because the gap is filled in with new bones and later on will be put on dental implant, they will be literally no relapsed in my bite as my maxilla now should be able to compensate my previously huge advanced mandible.

I tried searching videos about AMD but all i got is patient with cleft palate. Would like to know if any normal adult has done this procedure before.

I have only seen you discussed a lot about MSE in your channel. I am wondering if you have discussed about AMD or probably any similar terms of procedure that expand the maxilla forward instead of expanding laterally like in MSE.

Will get the expander installed by orthodontist tomorrow then on sunday will have the AMD surgery. Wish me luck.
