This is a great idea for quiz. Well done. I am a native English speaker and also an esl teacher. The past perfect continuous is a very difficult tense to teach because we often use the past simple or continuous instead.
I wanted to use this quiz in my class but there are a few mistakes which you need to correct.
2. The past perfect more appropriate in this sentence.
4. You forget the been
5. You can't say "had been tired^ in this context. Was tired.
7 past simple is better.
16. We would use "used to' or ^would' in this context (past habit)
18. The lately doesn't make sense in the sentence. Past continuous is more appropriate.

I think you are wrong because in past perfect continuous tense only had will I am talking about the fourth question
