ADVANCE HEALTHCARE DIRECTIVE MISTAKES

Your Advance Healthcare Directive is a legal and serious form that needs to be completed correctly, otherwise, it will not be legally executed and could be voided. Creating an Advance Healthcare Directive, which is a combination Power of Attorney for Healthcare and Living Will, could use up a lot of your time and emotional energy, so make sure that it is properly filled out the first time.

It's not too late as long as they are mentally capable if say they can't use their hand they can have somebody help them hold the pen and sign it as long as they are mentally aware of what is going on

Good morning, Sophia. Your advice has been TREMENDOUS HELP for me. Thank you for sharing this information. I am confused about something -- to initial the complex medical choices in Part 3 "Treatment Preferences" might not be a good decision 10 years from now. I want my agent to make those decisions based on my condition at the time. Is there a way on the form to omit initialing, but authorize my agent to make all the decisions that he/she knows would be best for me?
